Documents Needed for CauseMatch Pay Onboarding


This article outlines the documents and information your nonprofit needs to prepare before starting onboarding with CauseMatch Pay.
Having these details ready ensures a fast and smooth KYC (Know Your Customer) verification process and helps avoid delays caused by missing or mismatched information.

1. Signatory / Account Holder (Authorized Representative)

The Signatory is the individual legally authorized to represent your organization — typically an Executive Director, CEO, or CFO.

Required Information:

  • Full legal name (must match government ID)

  • Date of birth

  • Nationality / country of residence

  • Legal home address (no PO Boxes)

  • Last 4 digits of SSN (for U.S. citizens)

  • Government-issued photo ID (passport or driver’s license – clear image, front and back)

  • Email address and phone number

Best Practice:
✔ Use the same name and address that appear on your organization’s official registration documents.
✔ Avoid nicknames or shortened names, as Adyen will flag inconsistencies.


Required Information:

  • Legal entity name (exactly as registered)

  • EIN / Tax ID number

  • Registered business address (no PO Boxes)

  • Official website URL (optional but recommended)

  • Certificate of incorporation or nonprofit registration document

  • Recent bank statement (dated within the last 3 months) — must display:

    • The organization’s legal name

    • Account number

    • Bank logo

  • Routing number and account number

  • Short description of your organization’s purpose

Best Practice:
✔ Use a bank account owned by the nonprofit itself — not a fiscal sponsor, individual, or DBA.
✔ If operating under a fiscal sponsor, prepare a signed authorization letter from the sponsor allowing the use of the account.


3. Preparation & Technical Details

Before the onboarding link is sent, make sure to:

  • Verify that the organization name, bank name, and signatory name match exactly across all documents.

  • Disable any PDF password protection — Adyen cannot open encrypted files.

  • Use accepted file types: .jpg, .png, .pdf (each under 10 MB).

  • Complete the onboarding form in one sitting — it times out after approximately 30 minutes.
